At the website, look for the link for Flag Request or something similar. It may be listed under Constituent Services.

2. **Make Your Choice**

The form will offer several types of flags, in different sizes and fabrics (usually, either cotton or nylon). Flags are typically 3 x 5 feet, or 5 x 8 feet, though larger sizes are sometimes available.

The form will also ask for details such as the name of the person being honored by the flag, and the date you wish to have the flag flown (for instance, it can be flown on a person's birthday, or a special anniversary). You can also indicate the type of special occasion being honored.

The information will be included on a Certificate that accompanies the flag.
